1.Nature of Telehealth Care
Telehealth uses electronic communications (secure messaging, video, and asynchronous review of forms, photos, and lab results) to deliver clinical care without an in-person visit. Your Clinician may be located in a different city or state than you.
2.Benefits
Benefits include improved access to care, convenience, privacy, reduced travel, and the ability to communicate with a licensed Clinician from your home.
3.Risks & Limitations
Limitations include: no in-person physical examination; potential for technology failures, delays, or interruptions; possibility that information transmitted may be insufficient for clinical decision making; risk of unauthorized access despite security measures; and the possibility that your Clinician may determine telehealth is not appropriate for your condition and refer you for in-person care.
4.Alternatives
You may seek care in person from a local clinician at any time. Telehealth is one option, not the only one.
5.Voluntary Participation
Your participation is voluntary. You may withhold or withdraw consent at any time without affecting your right to future care. If you withdraw consent, we will stop providing telehealth services to you.
6.Records & Privacy
Your medical record is maintained securely and is governed by our HIPAA Notice of Privacy Practices and Privacy Policy. You have the right to inspect and request a copy of your record.
7.Communications
Messages between you and your care team are part of your medical record. Replies are not in real time. Do not use messaging for emergencies.
8.Consent to Prescription Treatment
If a Clinician determines a prescription is appropriate, you consent to having that prescription sent to a partner pharmacy and shipped to the address you provide. You have read or will read the medication’s safety information, including any boxed warning, and you understand that some prescribed medications may be compounded and prepared by a state-licensed 503A compounding pharmacy (see our Medical Disclaimer). You agree to follow your Clinician’s instructions and to report side effects promptly.
9.Emergency Care
Telehealth is not appropriate for medical emergencies. Call 911 or go to the nearest emergency room for chest pain, suicidal thoughts, severe allergic reactions, signs of stroke, or any life-threatening symptom.
10.Acknowledgement
By completing intake and continuing to use the Service, you acknowledge that you have read, understood, and agree to this Informed Consent.
